·         Section I: Please fill out the asset & liabilities section accurately including monthly payments where needed and describe details in designated section if you have any Other Assets or Other Liabilities (See instruction within brackets for designated sections)

Total in Liabilities Section should equal to Total Assets. (Net Worth =

Total Assets – Total Liabilities.

·         Section 2: Please include all info and fill out necessary boxes.

·         Section 4:  Please include all your personal properties and all the info in the boxes.

·         Sign and date with SSN.
